Addiction and Mental Health – Intake Services, Child and Adolescent at Nexus Business Park

Referral Pathway

Members of the public can contact intake directly. A professional referral is not required.

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Routine Referral: Referrals can be faxed to 780-408-8776. Internal referrals require a mental health assessment included with the referral, completed by either a Mental Health professional within Alberta Health Services Addiction and Mental Health or a child psychiatrist.

External Referrals should complete the Connect Care Referral form and provide any supporting documentation.

Urgent Referral: Urgent referrals will be indicated by checking off the “urgent” box on the referral. Please remember, this is not a crisis service. Urgent referrals are prioritized, but there is still a waiting period before the young person will be seen by a therapist.
